All wheels of 11th wagon no NR 91187 BCN from brake van and second wagon No CR 910018 all wheels of front trolley Pune end and two wheels of rear trolley SUR end derailed at 13.48 hrs.

The derailment occurred due to the combined effects of defective axle box, wheel gauge and centre pivot which caused obstruction to free movement of trolley while negotiating the R/H curve which was having slackness in gauge and excessive lateral wear of outer rail.

While leaving from Parasia loop line, rear trolley of coach no. NR 143509 GS derailed near point no. 18.

Entanglement of exposed spindle of buffer of culprit GS coach with the buffer of adjacent ACCW coach. and Variation in track parameters like Gauge, Cross level, etc.

All wheels of both trollies of motor coach no.3062b, positioned 2nd from front, of train no. 98864 Bandra-CSTM local derailed ahead pt. 102b, while approaching PF 2, CSTM harbour line.

Train No.12220 SC-LTT Duronto express was given through via main line at 02.12 hrs. ASM/MR reported train derailed in station blocking up & dn lines. Total 9 bogies derailed out of which 2 capsized.

Fracture of left hand lead rail of Up Main Line of Point No.109 B at km 584.652 under the passage of the train.

While the Train No. 95707 Dn.Mumbai CST-Kalyan Fast Local EMU (K-33) was on run between Dombivali and Thakurli stations, its 3rd Car derailed at km 48/18 by leading trolley from Kalyan end. Dn. line blocked. Up line clear.

Cross level variation upto 168mm was noticed, use of jack to lift the track under traffic condition is not permitted in suburban section. The excessive lifting resulted in cross level variation which led to derailment.

While Up NGC / BCN/ L Goods was approaching Adarkistation,16 wagons from 11th to 26th in the formation derailed in such a way that 9 wagons derailed and 7 capsized blocking through traffic of the single line section at 0455 hrs. However, there was no injury to any body.

Train No.12220 Up Duronto Express while on run between Yevat and Uruli stations of Daund – Pune section, one Tractor (MH-42Q-1880) dashed with it at Engineering, Interlocked, and MLC Gate No.10-B at km 229/08-07. No damage to Railway property and no casualty/injury to any train passenger.

Gateman of LC Gate No.10 opened the gate no.10 after putting back the gate signal to ON and LP and ALP failed to notice the change in the aspect of Up Gate Distant Signal and Up Gate Signal of LC Gate 10 and dashed with the tractor trolley resulting in casualties.

Train No. 50105 Dn. Diva-Sawantwadi Passenger while on run between Nagothane and Roha stations, its Train Engine + 04 Coaches (with T/E) derailed at km 136/08. Through communication blocked on section. Train Engine and 03 coaches capsized.

Weld failure.

System failure as per CRS’s preliminary report.

Train No. 96121 Thane - Karjat Slow Local EMU (TS-11) while entering on Platform 2 at Kalyan, one wheel of its Coach No. 2204A (8th from leading Motor Coach) derailed near Point No.137. 

Excess gauge, cross level variation on track, low joint, mud pumping, missing ERCs/ liners and broken sleepers were not attended in time.

Train No. 98052 Up EMU (PL-46) while arriving on PF 2 of Mumbai CST station from Up Harbour line, its 8th coach No.3034 C derailed with its Panvel end trolley.PF 1 and 2 blocked.

Excess gauge, cross level variation & poor maintenance of tongue rail & also due to sharp flange of a wheel of affected coach trolley and wheel dia difference on the same axle of the affected trolley.

Train No. 97136 (CK-4) Up Kalyan - Kurla Local EMU while leaving from PF No.1 at Kalyan station, its Leading Motor Coach (2089/C) derailed by one pair of wheels at point No. 118. Up Local line, Dn. Local line and Dn. Through lines affected. 

Defective tongue rail of Point No.118-1 and X-level variation.

Train No. 22865 Dn (LTT – Puri) Exp. while coming from LTT towards Vidyavihar on 5th line, at km 17/2 rear ladder of engine entangled with steel member of under construction FOB kept on cess resulting rear trolley of its front SLR No. ECOR 90716 (next to Train Engine) derailed by all wheels and dragged 38m after passing Signal No. S21 blocking 5th & 6th lines.

One of the columns, which were stacked parallel to the track slid due to train movement vibrations as the location of the stacking was slightly inclined & hit the LH rear side ladder of locomotive. Subsequently it got entangled with LH side foot board of the front SLR & caused it to derail. The accident occurred due to misjudgement in temporary stacking of FOB columns.

Train No. 96615 Dn. Mumbai CST–Titvala) Local EMU arrived Dombivili on PF No. 1A at 11.40 hrs. While starting from PF No.1A, its rearmost Coach No. CR 1050C derailed by CSTM end trolley blocking platform No.1A. Alternate lines available for the movement of Suburban services.  

Slack gauge of 62mm at 0 stn, 39mm at 1 stn, 22mm at 2 stn, 29 mm at 3 stn & 36 mm at 4 stn. At 0 stn, due to 62mm excess gauge.

Train No.97082 (K-92) KYN-CSTM UP EMU While leaving from PF No.1 of Kalyan station, its 06th coach No. 3082 C derailed by both trolleys.

Non replacement of defective reverse tongue rail (reconditioned on 14.03.2013) and non suspension of the defective Point No. 118-1 for traffic use after the previous derailment of 24.09.2014 combined with cross level variation coupled with a difference of more than 5 mm in the wheel dia of axle P2 of 6th coach.

100% fresh failure of the weld on the RH rail between Km 53/17 & 53/21.

As NI work was in progress at PEN stn for "Doubling", 12617 up departed from PF 2 on authority; and both trolley of 11th & 12th coaches and front trolley of 13th coach from Engine derailed at Pt. no. 118.

Improper fixing of non standard point switch clamp by inexperienced pointsman supervised by an inexperienced train Guard (working as site ASM incharge) giving way during the a passage of the train, resulting in the train taking two routes.

Train No. Karjat - Badlapur Special EMU Local while crossing from Up Main line to Down Main line PF No. 2 at Badlapur station, its Motor Coach No.CR 2197C derailed by 06 wheels (04 wheels of front trolley and 02 wheels of its rear trolley) while negotiating crossing portion of Point No.105, blocking Up Main line and Down Main.

Tight Gauge of 10 mm at 'O' station & 11 mm at '+1' station. Excessive gap in check rail.

Up BCN-DKAE goods train while approaching Dhalgaon station in MRJ-PVR section, its 2 rear most BCN/L wagon & brake van derailed one rear most BCN/L wagon +brake van got dis-coupled at km 504/02. Front portion 41 wagons dragged upto 500m and stopped at km 503/07, blocking S/line section.

Weld failure.

Train No. 12618 Up Mangala Lakshadweep Exp, while on run between Ghoti and Igatpuri stations, derailed. The train engine and 13 coaches derailed out of which, 03 coaches Capsized at km 143/03-05.

Due to bent of 10 mm in trailing axle of trailing trolley of Coach No. SR-12424 GS, 2nd from loco and rail fracture at point of drop

Train No.12809 Dn CSTM-HWH while on run between Hirapur- Chalisgaon stations, its one coach SE108436 GS next to rear SLR had a fire incident at km 320/18.

Due to the fire initiated from the inflammable materials, such as cloths, water bottles, Chappals, etc. of passengers kept on the bunk above the seat No.2 to 5 of Coach No. SE- 108436. Fire in these inflammable materials has got initiated by the still burnt (unextinguished)match stick/Bidi butts put by the passengers sitting on the bunk, negligently without realizing that the match stick /Bidi butts has not extinguished.

12 wagons of Goods Train No. Dn.VZP/BCNHL, while on run between Sonkhamb and Kohli stations in Amla – Nagpur section, sequence 41st, 43rd to 53rd from Train Engine, derailed at km 1000/16-12.

Train No. Up N/BOR Goods while passing through Bodwad station of Badnera – Bhusawal section, its 06 wagons (36th to 41st from Train Engine) derailed at km 474/29-31 blocking Up and Dn. Main lines. Through communication blocked on section. 

Due to "Wagon Defect" i.e. due to the obstruction to the wheel flange movement caused by the Equalizing Lever entrapped between LH Stock Rail and Tongue Rail of Point 103.

Train No. 95416 Kasara-Mumbai CST Up Passenger (N18 Up) while on run between Titvala and Ambivli stations, its 05 rear Coaches derailed at km 62/07-03 and uncoupled between 6th and 7th coach from rear, however the 6th coach from rear did not derail. Out of the 05 derailed coaches 01 Coach (303B, 2nd from rear) infringing Dn line. 

Due to breakage of Schaku coupler between coach No. 319C (NDTC) marshaled 6th from front and coach No. 307A(7C) marshaled 7th from front.
