Train 12810 up passed Talani station at 16.55 hrs. LP/ALP noticed fire/heavy smoke in loco, ALP fallen down from loco in running train. LP stopped the train at 16.58 hrs. Assisting loco demanded at 17.20 hrs. Assisting loco departed from DMN at 17.44 hrs. Train with assisting loco arrived to DMN at 18.38 hrs. Loco detached at DMN at 19.15 hrs. Train departed at 19.44 hrs.

Equipment failure due to poor maintenance by staff of SER.

Action taken- Fire and smoke detection system in non 3 phase loco proposed.

Train 12809 Dn was running on Dn main line, while entering on PF no.2 of IGP via middle line, its 9th (SE 098226 CN/S-13), 10th (SE 158802 WCB/Pantry car) and 11th (SE 028221 CN/S-12) coach derailed on middle line infringing Dn main line.

1-Combination of higher speed than permissible speed over crossover due to non synchronised operation between leading loco and banking loco.
2- Poor track maintenance.

Action taken- (1) High sensitivity digital VHF set procured.

(2) Work for improving communication in longest tunnel of both Ghat section using leaky cables and bidirectional amplifiers sanctioned.

Train while entering Khandala PF no.1, it’s all wheels of rear SLR CR99703 derailed at KM 123/22 on Dn main line.

Train 17324 Dn left PHQ station at 17.01 hrs. While running in PHQ-PC block section at KM 373/12-10, suddenly Two buffalo and One cow came on track and dashed with Loco no. 20021 WDP 4/ UBL of  17324 Up at the speed of 97 km/h, cattle guard broken and fell down on track and entangled with SLR no. SWR 14722 causing derailment of leading trolley of SLR.

Action taken-Counseled villagers through P/Way staff and RPF.

Train 51409 Dn Pune-Kolhapur passenger passed AKI station at 14.38 hrs. While on run between AKI and WTR section, it’s 3 wheels of front trolley of loco no 40574 WDP4D/PA derailed at KM 117/1-3.

Suspected Miscreant activity.

Action Taken- Locations identified by Engineering staff during festivals for Patrolling.

Train12859 Dn passed Sirsauli station at 11.55 hrs, while on run on Dn main line fire noticed in Power car no.SE 138853 LWLRRM of 12859 Dn by Gateman of engineering Gate no 146. Train Stopped between Sirsauli and Jalgaon station KM409/17-19 at 12.00 hrs. Fire extinguished at 13.30 hrs with the help of Jain irrigation Jalgaon and Fire brigade.

Short Circuit-

Due to short circuits in 24 volt battery supply system of Power Car.

Action Taken-Detailed instructions issued for maintaining and Operating power cars in addition to extant maintenance / operating instructions.
